Short Description of Instrument
Assessment of Life Habits (LIFE-H) adult version gathers information on characteristic life habits in the home, school, neighborhood and other environments.
 
The Assessment of Life Habits (LIFE-H) for children gathers information on characteristic life habits, in the children's various environments (home, school, neighborhood).
 
The LIFE-H includes two categories of life habits: daily activities (nutrition, fitness, personal care, communication housing and mobility) and social roles (responsibilities, interpersonal relationships, community life, education, employment and recreation).
Comments/Special Instructions
Inter-rater reliability is moderate to excellent in both the adult and child version. In the child version modest convergent validity with the PEDI and WeeFim were demonstrated. In the Adult version Validity and Reliability established SEM and MCD report in older adults with disability.
 
NeuroRehab Specific: The LIFE-H is a direct measure of activities of daily living and instrumental activities of daily living.
Scoring
For children: for ages 5-13 years there are long (198 items) and short (64 items) formats. Birth to 4 years has long format only (10 items). Adult version: the short form is 77 items and the long form 242 items.  Scoring is for two domains (degree of difficulty and level of assistance needed) and is based on 10 levels of difficulty (with or without assistive devices or adaptation scored 8 or 9), 6 levels of assistance needed (no assistance, assistive devices, adaptation, and human assistance, 2-7), also accomplished by a proxy (1) or not accomplished (0).Approximate time for administration of the short form child (62 minutes) and adult version (20 minutes).
 
Rationale/Justification
Although not CP specific, the LIFE-H has been used with individuals with CP. The LIFE- H is a useful tool for assessing wide variety of areas related to participation.
 
NeuroRehab Specific:
Advantages: A comprehensive tool that records a person's return to community life and participation after a neurological event.  Useful in studying community and family issues.
