Short Description of Instrument
Description: The Ages & Stages Questionnaires (ASQ-3 ®) is designed to screen children for developmental delays between one month and 5½ years.
Ages: 1 to 66 months old
Questionnaire Intervals: 21 questionnaires and scoring sheets at 2, 4, 6, 8, 9, 10, 12,
14,16, 18, 20, 22, 24, 27, 30, 33, 36, 42, 48, 54, and 60 months of age; approximately 30 items per questionnaire
Language Availability: English and Spanish
Comments/Special Instructions
Administration: Parents/caregivers complete questionnaires. Can be completed at home, in a waiting room, during a home visit, or as part of an in-person or phone interview.
Completion Time: 10–15 minutes
Areas Screened: Communication, gross motor, fine motor, problem solving, and personal- social.
Software Available: None
Scoring
Scores: 5 raw scores, one for each area.
Scoring Options: Professionals, paraprofessionals, or clerical staff score the questionnaires.
Rationale/Justification
Reliability: Strong test-retest reliability (.75 – .82); intraclass correlations by area range from .43 to .69; internal consistency ranges from .51 to .87.
Sensitivity/ specificity: Excellent sensitivity (.86) and specificity (.85)
