The instrument is available here: Parkinson's Disease-Cognitive Rating Scale.
 Supplemental: Parkinson's Disease (PD)
Short Description of Instrument
The Parkinson's Disease-Cognitive Rating Scale (PD-CRS) was developed to "capture the whole spectrum of cognitive functions impaired over the course of PD." (Pagonabarraga et al., 2008)
Construct measured: Global cognitive function
Generic vs. disease specific: Disease specific
Means of administration: Rater administered (paper and pencil)
Intended respondent (e.g. patient, caregiver, etc): Patient
There are 9 items and 2 domains – subcortical-type cognitive items and cortical-type cognitive items.
The nine items include:
    • Immediate free recall verbal memory
    • Confrontation naming*
    • Sustained attention
    • Working memory
    • Unprompted drawing of a clock
    • Copy drawing of a clock*
    • Delayed free recall verbal memory
    • Alternating verbal fluency
    • Action verbal fluency

*Cortical-type cognitive items
Subcortical (0 –114) and cortical scores (0 –20) are obtained by adding the raw scores of the items within each group. Total scores (0 – 134) on the PD-CRS are calculated by adding the Subcortical and cortical PD-CRS scores
Higher scores indicate better cognition.
