Short Description of Instrument
Parent report tool for parents of young children that measures participation, specifically what the child does through the day. Reliability and validity have been tested in children with CP. Internal consistency is high. Test-retest also tested with acceptable reliability.
 
Comments/Special Instructions
CP only-ages 18 months to 5 years 11 months. Internal consistency of the domains of the measure was high, Cronbach alpha values ranging from 0.86 to 0.91; test-retest for participation in family and recreational activities was acceptable, ICC = 0.70, and in self-care was high, ICC = 0.96 (Chiarello et al., 2014).
 
Scoring
Eighteen item questionnaire Two domains: Family and Recreational Activities and Self-care. The family and recreational activities domain has 11 questions scored on a 5-point Likert scale from 5=very often to 1=never. The self-care has 7 questions, scored on a 5-point Likert scale that distinguishes whether the child is able to consistently perform the activity and need for physical assistance of an adult, 5=yes, 1=no, unable. Raw score for each domain is converted to an interval level scaled score from 0–100.
 
Rationale/Justification
This tool was initially pilot tested with the parents of 6 children who had CP across GMFCS levels I, III, IV, and V. A subsequent study using a test-retest approach with 387 parents of young children with CP 18 months to 5 years of age demonstrated differences in children by GMFCS; children in GMFCS groups I – III demonstrated more change for the self care domain (moderate effect) than in the Family and Recreational Activities domain (small effect). Parents of children GMFCS levels IV – V reported a small effect across both domains (Palisano et al., 2014).
